Trật gân chày sau là một tình trạng bệnh lí hiếm gặp dẫn đến dễ bị chẩn đoán sót hoặc muộn. Đa số tác giả cho rằng việc điều trị bảo tồn không đem lại kết quả tốt bằng phẫu thuật nên việc chẩn đoán đúng để đưa ra chiến lược điều trị là rất quan trọng. Khai thác bệnh sử, thăm khám lâm sàng kết hợp các phương pháp chẩn đoán hình ảnh giúp tránh bỏ sót tổn thương. Chúng tôi báo cáo trường hợp một bệnh nhân trật gân chày sau được chẩn đoán và phẫu thuật sớm, bước đầu cho kết quả tốt.
