RESULTS OF THE LAPAROSCOPIC APPLICATION WITH CHOLEDOCHOSCOPY AND LITHOTRIPSY IN THE TREATMENT OF BILE DUCT STONES AT MILITARY HOSPITAL 175

Background: Choledocholithiasis are common pathologies in Vietnam with the current treatment strategies, the recurrence incidence is still high. In recent years, laparoscopic cholecystectomy with reflexible choledochoscopy and electrohydraulic lithotripsy has been the preferred technique for difficult common bile duct stone removal. This procedure will improve the recovery time and reduce stone missing rate.
Objective: Clarify the clinic, para-clinic feature, and the results of laparoscopic choledochotomies to management bile duct stones. Materials and methods: A cross-sectional study of patients with hepatolithiasis and/or choledocholithiasis who underwent laparoscopic choledochotomy using intraoperative cholangioscopy at Abdominal Surgery Department, Military Hospital 175, from June 2021 to June 2024.
Results: A total of 54 patients were analyzed with an average age of 58.6 (28-83) years; the female/male ratio was 1.8. History of biliary tract stone surgery was found in 10 patients (18.6%). Average surgery time was 109.9±31.3 (60-200) minutes; laparotomy conversion was 3.7%; the stone clearance rate was 84.6%, and the average hospital stay is 7.7 (6-25) days; the rate of intraoperative and postoperative complications was 1.9%; 5.8%, respectively.
Conclusions: Management bile duct stones by laparoscopic choledochotomy was feasible, safe and effective with high complete stones clearance, low intraoperative and postoperative complications.
