ASSOCIATION BETWEEN NEUTROPHIL-TO-LYMPHOCYTE RATIO AND CORONARY ARTERY LESION SEVERITY ASSESSED BY GENSINI SCORE IN PATIENTS WITH ST-SEGMENT ELEVATION MYOCARDIAL INFARCTION

Objectives: To investigate the correlation between the neutrophil-to-lymphocyte ratio (NLR) and the severity of coronary artery disease (CAD) assessed by the Gensini score in patients with ST-elevation myocardial infarction (STEMI).
Subjects and Methods: A cross-sectional study was conducted on 109 STEMI patients at Military Hospital 175 from September 2025 to April 2026. NLR was calculated from complete blood count at admission. The anatomical complexity of CAD was quantified using the Gensini score based on coronary angiography findings.
Results: The median NLR was 5.29 (IQR: 2.82 - 7.38). The median Gensini score was 53 (IQR: 36.5 - 80.0). The high NLR group (≥ 5.0) had a significantly higher median Gensini score compared to the low NLR group (< 5.0) (77.75 vs 42.50; p < 0.001). Spearman correlation analysis showed a moderate positive correlation between NLR and Gensini score (r = 0.571; p < 0.001).
Conclusion: NLR is positively correlated with the coronary anatomical complexity according to the Gensini score. NLR can serve as a simple, cost-effective biomarker to predict the extent of coronary lesions upon admission.
