Abstract

Objectives: This study aimed to evaluate the relationship between maternal age and chromosomal aneuploidy in day-5 blastocysts from in vitro fertilization (IVF) cycles with preimplantation genetic testing for aneuploidy (PGT-A) at An Sinh Hospital in 2023, and to analyze correlations between maternal age, number of mature oocytes, fertilization rate, blastocyst development, and aneuploidy rate.

Materials and Methods: A retrospective cross-sectional study was conducted on 490 IVF patients who underwent trophectoderm biopsy and PGT-A for day-5 blastocysts. A total of 2,472 embryos were analyzed using next-generation sequencing (NGS, Illumina, USA). Patients were divided into three age groups: <35, 35–40, and >40 years. Statistical analyses were performed using R software and Microsoft Excel, applying Chi-square tests and multivariate logistic regression to assess the effects of maternal age, number of retrieved oocytes, and number of day-5 blastocysts on aneuploidy risk.

Results: AMH levels, fertilization rate, and day-5 blastocyst formation rate decreased significantly with increasing maternal age, whereas the number of retrieved oocytes and mature oocytes (MII) did not differ significantly among the age groups. The euploid rate decreased from 36.24% in women <35 years to 26.08% in those aged 35–40 years and 20.63% in women >40 years, while aneuploidy increased correspondingly from 20.86% to 33.27% and 41.27% (p<0.001). Multivariate logistic regression identified maternal age as the independent factor most strongly associated with aneuploidy among the variables examined (OR = 1.052; 95% CI: 1.031–1.073; p<0.001), while the number of day-5 blastocysts showed a protective association (OR = 0.965; p=0.002).

Conclusion: Maternal age significantly affects ovarian function, fertilization capacity, and embryo chromosomal quality. AMH levels, fertilization rate, and day-5 blastocyst development decline with age, whereas the number of retrieved and mature oocytes (MII) remains relatively unchanged. Maternal age is the strongest predictor of chromosomal aneuploidy, while the number of day-5 blastocysts shows a protective effect.
